Is anyone having issues with the GE html immunization form missing a place to document the date the VIS was given to patient. VFC auditors informed our practice the form does not meet CDC requirements.
I was wondering how other practices are documenting the date the VIS was given to the patient, or how they provided the VIS to the patient.
We placed our VIS documents in Handouts. When we print out the handout and click on the "Record handout printing in Chart" checkbox, it will create a document that notes what handout printed, who printed the handout, and the date/time the handout was printed.
On the form, there is a section on the bottom left... "Questionnaire." The last question in the questionnaire is: "Vaccine information given and explained to patient?" Your staff needs to check "Yes" on that question.
