Yesterday at around 3PM all of our system users began receiving flags from System Maintenance.
We received hundreds and hundreds of flags: One provider reported they had about 10 flags on their desktop and when they came back 15 minutes later there were 792 flags on their desktop.
The messages all say something like
"The following immunization, recently entered on this patient's chart, has been inactivated by the CDC:
122 - rotavirus vaccine, unspecified formulation
entered: 2016-02-23 15:18:40
All vaccinations have been sent to the registry. The inactive vaccinations may not have been recorded correctly by the state immunization registry
CDC Notes - No notes available.
For more information, refer to http://www.cdc.gov/vaccines/programs/iis/code-sets.htm l"
But if I look at the record, the child didn't receive ANY immunization on the date/time...not even close...we hadn't seen this child since last summer.
My Qvera export file for immunization registry is usually around 100 KB. This morning's file is 107,000 KB.
I was actually out the office when this began. I haven't done any work on immunizations since our last upgrade to CCC v9.1 and basic 1.3 in December.
There is no evidence that immunizations were entered again..no duplicates in the database yesterday.
Any ideas?
I have been getting this exact message for over a year now and GE doesn't seem to know where they are coming from, are you in Texas by any chance? It has something to do with your immunization registry submission
I believe we got this when people were doing their actionable reminder letters and accidentally chose immunization registry in the inquiry drop down vs what they normally chose. We found by chance conversation with a practice manager and timing was exactly right. That is why it seems random. There is no permission restricting users from accessing that choice in EMR (great right?) but there is a setting in Qvera to stop sending a flag for that - I just need to remember where we set what. I had conference calls with GE and Qvera and they couldn't figure it out but I let them know what happened when we found it. I'll update this post with our SR and/or setting in Qvera to turn flags off. Thx. Jen
