I'm in a FQHC using FTCA for malpractice coverage. We are required to track our response time as we communicate with patients about lab and imaging study results. We need to capture the Order date, when the result returns to the office, when the Provider reviews the result, what catagory the Provider classifies the result (Normal, Abnormal, Critical), and when the patient is notified (Phone or Mail). We have different response time goals based on which of the 3 classifications the result falls into.
We also need to document the Provider's instructions for follow up and Support Staff''s completion of the task.
Any FQHC's out there with a form to capture this data and a report to track it?
