A couple of my clinicians have been using the customization features of a CCC for to generate a preventive care form. Before we re-invent the wheel, I'm wondering if anyone has taken the time to make a good comprehensive form to satisfy the need.

Here is our Adult Prevention form
It has AAA, DEXA, Mammo, PAP, Colonoscopy, Lipids, Aspirin and various Immunizations.

I dont mean to hijack a thread but how do you post forms? I went to the beginner forum help section and tried to look it up, the instructions there indicated there is an "upload attachment" button below the reply text field and above the "Smileys" section, but I dont see it...

Look at this post that ErnieT did. I is a good walk through with screen shots. I did a post with a video but it isn't working since the upgrade. If you still have issues, let me know.
