I'm interested to hear what other sites have done to meet this measure requirement. We use the prompts with the CCC CPOE forms (e.g. CHF diagnosis), but also have users on strictly VFE encounters.

We used code in VFE to prompt the provider if the patient was a smoker in the form of a pop-up. It said, "This patient is a smoker, did you provide cessation counseling today?". Yes or No button. The response was then recorded in the user yes/no audit. In that form, we also put a handout button if they wanted to print educational materials.
We did something similar to Angela's practice, but we did it with the BMI. We set up visibility to flag the provider for various ranges of BMI (color-coded for underweight, normal, overweight, obese, and morbidly obese). Depending on the range, we also provided a checkbox to document counseling and a button for educational materials.
