I am checking to see what everyone is doing for their Medicare Wellness visits. I have created a couple of text components that the providers can use to document these and wonder if anyone has a specific form they utilize or something custom that someone would be willing to share!!

We have 10 PCP providers and see a good many medicare patients, so we created a "wellness team" of a couple of MA's. The ma's run a report from sql reporting each morning looking at the next day's schedule. This gives them a list of medicare patients that should be eligible for either their initial wellness visit or a follow up wellness exam. After verifying that the patients are eligible(there are a couple of things that aren't reportable) they send a list to our scheduling department who call the patients and let them know that we would like them to spend about 10 or 15 minutes more with us.
I created a completely separate visit with forms to capture the info required for wellness exams. We've been very successful with getting wellness exams with this process after we tried several other ways.
You could also try corecaresolutions.com. Since a lot of the information needed in a wellness visit is patient driven they have developed a simple patient encounter for IPADs that you give to the patient and they can fill out the info ahead of time.
