Was wondering how other practices handle mental health/HIV dx on problem lists? We do have our behavioral health documents as confidential but the diagnosis goes to the problem list for others to see. How are other clinics handling these diagnoses on the problem list?
This has been an issue since we started with the EMR. We are multi-specialty including Psychiatry. We have the office visit documents as confidential, but the problems do show up on the active list as well as any medications and the appointments are in registration. The majority feel that everyone in our clinic signs a HIPPA form and other providers should be aware of all the patients medical problems and medications. The idea was suggested to have the Psychiatry problems go inactive after 1 day, but if you search inactive they are still on the list. We have not had an issue with our current workflow.

In our clinics, we put a pop up on the chart that states "Printing Precautions". We were more concerned about releasing records with this info to other facilities than protecting it from our own employees. This pop-up is to alert anyone that prints records to send to insurance company, workers comp notes, etc that they must manually remove (strike through with black marker) any diagnosis that should not be released. Not a good solution, but once we got folks educated on the workflow it has worked for us.

In your office who is allowed to see the Mental Health/Psych notes? Do some have complete access while others use access on demand? Are some document types blocked for everyone except the mental health provider who does the visit?
