Good morning All,

Our location is looking to use the Million Hearts Model Initiative: The Million Hearts® CVD Risk Reduction Model supports prevention of CVD, improved health outcomes, and health care cost savings through systematic implementation of beneficiary risk calculation and stratification.

What I am looking for is a possible workflow or forms that would best suite our needs and data. Looking for other sites that are using this initiative.
