Does anyone have a PQH9 form for the GE portal that they would like to share?

Does the GE portal allow for forms (such as a PHQ-9) to be filled out and entered by a patient? if so, I would love to learn how to do this as well. (sorry I don't have what you're looking for mate)

I don't see why you could not do this on the Portal if you would like to. My thoughts would be make a questionnaire that save the answer to each question to a "EMR Observation". Example question 1 to obs term "PHQ9 Q1". This would collect your answers. Not sure you could score on the portal, but you could when you got notification of form had been submitted then go to a created form from say PHQ9 encounter. This encounter could be just a clinical list update. Have that form pull previous obs values and then calculated and save to PHQ9 Score observation.
An example of a form that saves to obs terms is the BP readings form, if you need an example.
I believe that would work.
