Good Morning,
Does anyone know if Athena has their own PRAPARE form that can be downloaded? Any feedback would be greatly appreciated.

This is for Athena Practice and Flow
We are a small rural clinic with 2 providers and a new BHC.
We were able to have our VAR support go to the https://www.nachc.org/prapare-templates/. Down toward the bottom is the "Centricity form". We have been trying to use it for several years but because of a grant we are to trying to work it to our BHC visits.
