What products are out there for patients to complete medical history forms on tablets? We are aware of the product offered by esig but are wondering if anyone else knows of any others to contact?
We are currently using a product called Instant Medical History (IMH) it works well when it works. We have had trouble with it since upgrading to CPS 11. I have one provider using it and he loves it, so he has a workaround until it is up and running as it should. Once we have our problem figured out, I have other providers wanting to use it. Patients can enter information from home or at your location of care.

There is DocuSign this is done through Kryptiq. The new version coming in the next couple of weeks and will allow for bidirectional obs terms. We currently use Docusign via e-mail to send new patients history forms. However it does not contain obs terms, so staff still transfers the information into the chart.

+1 for DocuSign. I have been with DocuSign since their early days and been a regular alpha/beta tester for their product releases. The support is fantastic too.
We currently use a low-fi entering form which resembles a Scantron sheet from high school. The patient fills out the information on Scantron sheets from multiple choice questions, for say the PMH or PSH, with a no 2 pencil. These sheets are then scanned into the computer system and they populate live in the patient's chart. They can be over ridden and added to, in the EHR. The patient can also fill them out on our portal ahead of time to save time in the waiting room.
Health e Signature also has a pretty iPad product they regularly show at the CHUG. We've seen a few demos but haven't purchased.
Essentially, you take your Encounter Forms and push them to the patient on an iPad. Once completed, the data comes back into your form, within the open Office Visit note. http://www.healthesignature.com .

We use this one and it has been working great. We have been live about a month
