Does anyone have a report they would be willing to share for pulling a provider's patient panel?

The biggest difficulty I find is in pinning down exactly what the person requesting the report means when they say "a provider's patient panel". I get a very wide range of definitions.
I'm in the process of writing an article on this for my Adventures In Centricity blog, and it will include a stored procedure and two different versions of the report. I'll post an update here when it's available. Maybe it will get you pointed in the right direction.
