Currently our Registration Staff has permissions to sign Phone Notes as they are sometimes the last contributor to a note. Unfortunately this permission also allows them to sign clinical list changes, which can include Rx's (aside from electronic). Not what we want to happen. Typically they would not get the note routed back to them until Rx's have been signed but today we had a situation where Registration needed to add to the phone note that included pending "Print and give..." Rx's. When she went to put it on hold it signed the clinical list changes. I have instructed her to uncheck the Sign Clinical List changes box but there is concern that Registration staff has the ability to sign Rx's within this document type.
Curious how other facilities handle their permissions with Phone Notes??
These are the options I have provided to the managers as a resolution to this concern:
Remove signing permissions for Registration staff (which could interrupt workflow)
Have Registration staff be sure the Sign Clinical List changes box is unchecked (still leaves the permission to sign Rx's intact...)
Make sure Nurses/Providers do not enter Medications (new or refills) within that document type (could interrupt workflow if a provider has to view the phone note and Rx refill)
Usually we do not put Rx into a phone note but create a sperate document noted as RX refill. This way you wouldn’t have this issue and you can reference the visit doc is if needed in the phone note.
