Once before I remember seeing something about place where in setup you link the Phone Nt icon to the actual encounter/document. Does anyone know where this setup is done?
And a somewhat related question - can we create/add other icons like the Phone Nt? To speed up certain processing, we would like to add another icon or two.

Not sure this will help you but you can add the phone note form to a document template under Settings > Chart Documents > Document Templates. Every time an encounter that pulls that document template is opened the phone note form would be a listed form.
We have never used the standard phone note. Before we went live, GE set up our revised phone note to be used. Unfortunately, that was in 2004, so I don't remember exactly how it was done. However, I think it was done as Lisa suggested--by modifying the Phone Note document template in the Enterprise folder.
