How are other practices capturing data prior to the patient's office visit? Does anyone use the patient portal or their website to get these forms completed and what is the workflow for getting the patient's chart updated once they are filled out?
I am actually getting close to beginning this very project.
Rough plan ==
Run report of next week's appointments (in Crystal), saving to Excel
Use the Mass Mail add-on to Portal to securely email
probably with some nice merge of doctor name, etc...
direct users to complete a form on the Portal
Portal would write to specific OBS codes - take a look for PED (patient entered data) examples
incoming document to be auto-signed by Dr. Portal - making OBS available
During the beginning of the visit, the MA rooming patient would see on part of custom VFE form that some PED data had been recently updated.
MA inquire with patient, confirming entry. If all ok, then a button the the VFE form to copy PED Obs Code data to real Obs Codes.

that is quite a broad question... there are a range of portals you can use, Medfusion and Kryptiq (how I believe was just bought out and changed their name, ELNI if I recall correctly). These can also help with online patient payments.
You can use anything from, paper forms that get scanned into the system link scantron sheets, to kiosks, and tablets. I'm sure there is a BYOD solution as well.
Whichever solution you choose, they all integrate through various interfaces with CPS/EMR to enter the information the patient fills out for you.
Interestd in hearing how folks are getting portal information into the patient's actual charts. Not easy from our workflow and pretty clunky.

We are going to start trying something similar to Joe. The only difference is we are going to have a couple of providers testing out virtual visits at the same time. The patient will be prompted to go to the portal and complete a form there prior to connecting. The data will come over as specific patient entered data obs terms which the provider will see on the form. They will have the option to pull the values into the day's visit or ignore.
