We are using Relay Health Patient Portal and have started getting calls from patients about their medication list. It seems that the med list that they view on the portal includes all medications without a stop date. If our providers CHANGE a medication, the patient sees both the original entry and the changed entries on their med list (becasue there is no stop date for the original). Does anyone has a work around that doesn't involve the provider having to remove the med and then add it again? Relay Health apparently has said that they can't/won't be responnsible, etc, etc. But this is causing some confusion for the patient. I haven't worked on the portal project but said I would ask the CHUG for suggestions or ideas.

We do not user the Relay Health Patient Portal, instead we use the SureScripts Patient Portal, but we did have a similar issue with a product we were using. We have a process that sends all of our patient data into a warehousing product called "i2i", so that we can build and run reports off of large amounts of data without impacting our Centricity performance. When we first started using i2i, we too had all medications showing up in reports, regardless if they were active or inactive. Initially, i2i thought that they could just filter out medications that had end dates that had passed by, but as it turned out several providers liked to just stop a medication without ever putting in a end date, so they had to filter out stopped medications regardless of end dates.
If your providers are at least putting in end dates, I still think you should try to get Relay Health to work with you to filter out medications that have end dates that have passed by. If not, is this data sent to their patient portal or is their patient portal tied directly into your database? If it is sent to the patient portal, can you look at filtering the data as it is sent? If nothing else works, maybe time to look for a different Patient Portal solution, who knows if you mention this to Relay Health, maybe they might be more willing to work with you at the risk of losing your company as a customer.
