We just implemented the SureScripts portal, after leaving Medfusions behind. We are now very disappointed with the patient communication workflow:
- If we use the chart-note based message, you can only go back & forth with patient once, or the additional messages on the thread go via secure messaging, and the info is scattered.
- If we use secure messaging solely, the clinician can't tell from the EMR desktop if they have any messages. Messages are divorced from actions (the secure message is one place, and a script, referral, etc. has to be done another place in a different note).
- If we use the Outlook notification system to tell clinicians they have a secure message, they won't see it from EMR in real time. later on (evening, next day) they can't tell from the Outlook message whether they already handled the issue or not.
All the workarounds we see are very clumsy. Anyone develop custom forms that use SureScripts functionality and make this more elegant?
We have patient messages come into GE as documents. Easy to manage for us, but confusing for patients since they do not see sent messages.
As long as the reply (by nurse or doctor) has the box checked for 'reply to chart' then a patient response will also show up for the doctor as a GE document.
We are also testing out the automation tools - mass mailing, auto mailing - that go with the Patient Portal. With some thought and work, we are slowly getting some value out of the Portal - as opposed to simply being able to check a Meaningful Use box that we have a Patient Portal.

I feel your pain and had a long conversation about this at the last C-live/CHUG meeting with Surescripts. There has been no follow up since then, but maybe if enough people bring it to their attention it will move it up on the list. It is difficult for staff to keep track of conversations that take place in secure messaging. There needs to be a better transition between the chart/EMR and secure messaging.
