Is there another way to save and create a TOC other than in a referral order? We have always done it that was as it was part of our referral workflow but we need to be able to create a TOC and don't need it to have anything to do with a referral.

Not sure what you are trying to do. But, when you click [Update] and select an Encounter Type, there is a checkbox for 'Encounter is a Transition of Care' just above the OK button.
Also, when closing the Encounter you will see another checkbox (same function) for 'Encounter is a Transition of Care'.
This could be a way of indicating that a visit should be considered a Transition of Care - especially if you are working to achieve MU status for Med Rec at TOC.
I just asked this question to GE and this is the response that I got:
You need to give the user Security to create a CCDA under Admin>User and Resource Management>Users>User Management.
Edit the user account and go to the Security tab. Under Main Menu> Chart> you should give them Export Summary Documents permissions.
This will allow you to go to the patient chart and the office visit, right-click and Create Clinical Visit Summary.
I would then save to chart and close. (Make sure you wait to do this until the document is signed, because if you print the Clinical Visit Summary from a button
On a form, you will not get all of the clinical list changes.)
Once this is done, you can send it via secure messaging, just like the Transition of Care document.

We are moving away from Centricity and trying to use the TOC as a way to import problems, meds, allergies into our new EHR so we need to be able to create and save a TOC. The way you described doesn't show the TOC as an append to the document.
Any other ideas?

If you are looking to dump a TOC/CCDA for all patients into a new EMR, I would think you would want to automate this and not manually export a TOC for every patient manually.
It is possible to setup a Qvera or Mirth interface that would generate a CCDA and save it somewhere. If you would like assistance setting up an interface for this please let me know.
GE has a tool that can be used in the Report/Inquiry area that will generate xml CCDA's for all patients giving criteria selected. It is about $500 to implement. That may be easier than setting up an interface, or is that really a simple thing (with your help)?

We were in the same boat- moving from Centricity and needed to create a mass group of TOC's. We paid GE to set this up for us and it worked well to enable us to do an Inquiry to export patient CCDA's in bulk. In our new system (Meditech) we import some of the items (like meds).
