Hello,
Just hoping to see what other sites are doing for patient reported vitals obtained through telehealth visits. We are doing both virtual check ins (audio only) and Telehealth visits (audio/video). For now, we're having staff document patient reported vitals in HPI since entering patient reported vitals in the Vital Signs form would be indistinguishable in the flowsheet and might affect quality. I was going to go looking for some patient reported vital sign obs to add documentation fields to a form.In the long run, it would be better to have patient reported vitals be discrete data, rather than just free text but we want it to be distinguishable from site measured vital signs. Just curious to see how other sites are handling this.

Check out the Telemedicine V1.2 form on CHUG Marketplace. It has section that is used for Patient Reported Vitals and they record to OBS terms specifically for vitals reported by patient. You could use that form for inspiration.
