Hello,
Effective this year, CMS now pays higher for transitional care management
services provided to patients within 30 days post discharge from a hospital
setting. We are venturing into a pilot study to see if it
is worth it for us to bill these new transitional codes vs. the usual E&Ms.
The billing for these services is somewhat complex in that you can't bill until
day 30 and you must document a sequence of services through-out the 30 day
post-hospitalization period. I am wondering if
anyone has developed a form that captures the required pieces of
documentation for CMS, and if so, if you would be willing to share that with us? The transitional care management codes are 99495 and
99496. Any information would be appreciated. Thank you !!!
