We are a multispecialty group with 3 specific departments that share patients. Patients who see providers in more than one department have their medication lists reviewed at each visit by the clinical staff in that department and the medication list is corrected based on the patient's input. Our dilemma is that patients will misinterpret the question and will tell the nurse or MA that they are not taking a medication when in fact they are still on that prescription but may not have taken it today or may need to get it refilled. The nurse or MA remove the medication from the list and then when they return to one of the other specialties that prescribed the medication there is a frustration level because the medication is missing from the active med list. Also there is no way to reactivate the medication once it has been removed. Does anyone out there have a workflow that avoids this issue? Or has anyone figured out a way to re-activate a discontinued med?

As non-ideal as it is our users are instructed to not remove medications that are prescribed by other providers in our organization without the consent of the prescribing provider (usually via flag)...
That is a great thought but then how do you deal with the accuracy of your medication list if the medication really has been stopped by the patient? Also when is that flag sent to the other provider and do they respond before the patient cpompletes their visit?
