We use patientlink bubble forms. We have 4 practices using software (family practice, neuroscience institute, rehab medicine and Breast surgery associates). we either mail, email or have the patient fill out the forms when they arrive. They are scanned into a special scanner and it imports as a signed document directly into the patients chart. All data is recorded as structured observation terms.
The only issue we occasionally run into is that the form is not scanned in prior ot the office visit being started so the discrete obs terms/data does not populate for that visit. This is a workflow issue.
Other than that the MDs say they can not function without having this solution, it inputs all the medical history information (PMH, PSH, FH & SH) for them to simply review with patients. The company is very easy to work with and flexible. I highly recommend them.

I have been planning for a while to incorporate data from patients into the EMR. By the way, GE OBS tables have lots of fields noted as "PED" (or similar) for patient-entered-data.
My team will have it first meeting shortly, since this will be part of a larger project. But the idea is to request that patients complete some on-line forms through the SureScripts patient portal prior to an office visit. And the MA Intake form will display an indication (and possibly some of the data) that a patient has pre-completed data. The MA would verbally verify that data was supplied, and then with a button push the PED data would be copied into the system.
Still working out details. And I know it would be possible to design an auto-sign process so that the data would not need to be accepted. Just unsure if we want a button push to accept.

We use SMPP currently but we saw a demo for Medfusion portal today and one of the only advantages I saw was that it would send in CCDA data to the patients charts for data that the patient filled on the portal out which would allow you to run a reconciliation against it.
We are members of CQIC ( http://www.clinicalcontent.com/) - we do not use their CCMe product, but other members do and have great things to say about it. My understanding is that you can build your own or use pre-built entry templates that correspond to what patient needs to fill out. Front desk staff can send those templates out to patient when they arrive or before the visit. It's powered by Twistle. John Janas et al should be at CLIVE in May with a CCMe booth.
We use PatientLink bubblesheets for in office and have e-forms on the patient portal.
We use Surescripts SMPP and I've set up all the forms with GE OBS terms. When the staff goes to a certain form (say ROS form) - there is a "view/import" button that they can push and view the data and decide to insert it into the note.
I've set this up for our ROS, PMH, PSH, Smoking status so far. They work great for our workflows!

I am so glad someone else has asked this question. I have wondered why more people are not using this type of technology. I have been using Instant Medical History for the last 3 years. Patients are able to complete their history by entering their reason for visit and the program will use branching logic to delve deeper into whatever complaints that they have. You can enter more than one complaint. For new patients, it will ask a thorough PMH, PSH, FH and SH along with the ROS.
The data comes directly into Centricity through Linklogic. It produces a form of OBS terms and also a well organized transcript that shows up within about a minute of them completing their questionnaire, so it is available right away.
To bring it into a progress note, they supply a free form that allows pulling that patient entered data in with a simple click. The info can then be edited as needed. I do think the Centricity form could use some updating to make it function better as not all of the history obtained has a corresponding place to pull it over. Therefore, I still have to do some copying and pasting of the information at times. It is a matter of matching OBS term with the form. I think if more people were doing this, a more complete form could be developed.
The biggest challenge has been in training patients and staff on the different work flow. We encourage patients to complete their history at home before their visit, but also have computers at the clinic they can use.

We also use PatientLink bubble sheets and Surescript's patient portal. We LOVE it when patients complete their information via the portal - it makes check-in much faster. Because of this, I am currently developing a workflow for patients, at the time of check in, to complete the necessary forms on the portal via an iPad. Ideally, we could eliminate the need for the bubble sheets although I think we would always have them around just in case 🙂
