I am looking for a good workflow that would ensure that a service order is placed within an Office Visit document. Our providers tend to forget to place a service order, then end up creating duplicate tickets in billing because they forget to adjust the clinical date and/or location of care on the Append.

Are you referring simply to the "Office Visit Level 3" or "Office Visit Level 4" or ....?
Or, to orders for labs and other services?
I would like the providers to be "required/forced" to add a service order (E&M or any procedure performed in the office) before they sign the office visit. They should not be able to sign an office visit document until the service order is placed.

A couple of thoughts:
Embed the ORDERS_NEW function into your VFE that your providers use to do their office visit; checking to make sure that the type=S returns a valid code.
(I thought there was a way to do this, but cannot seem to find my notes.) Amend your VFE that the doctors do for their office making a radio button (or similar) way to choose the service type. And make this a required field to close the form.

Unfortunately we only use GE provided CCC forms not VFE forms.
We are in the process of testing a VFE form with button to enter service orders (E&M, Therapy Modalities, Injections, etc.) . Our plan is to start in our Therapy Department before pushing to the clinic.
If you don't have any custom forms, your best bet may be to use the MU Core Checklist form that is in the CCC content. At least it shows a red Missing! if there are no orders documented. Just going to have to train your providers to make sure that box shows a visit order.
If you have the full CCC suite, check out the Test Management form. It has a Billing/Services tab that allow you to set up the form to add the orders for office visits as well as other procedures. I am not sure if this is part of CCC Basic, but I don't think it is.
