Anyone have a Advanced Directive Form to share?

I am looking for an Advanced Directive Form if anyone is willing to share. I had one several years ago, but I can not find it.

Advanced Directives vary by state, so we print off the one for NH for the patient. Once they fill it out and have it notarized we scan it into their chart. They are also encouraged to bring a copy to the hospital we are affiliated with.
