Anyone willing to share their current workflow(s) on this measure? We had this one working a while back, but now results are sporadic and we can't seem to incorporate a functional workflow for our providers.

We have built an encounter form that serves as a dashboard for these quality measures. It looks at the blood pressures, diagnosis of hypertension, and alerts the provider to the status - second hypertensive, prehypertensive, etc. It then makes recommendations to the provider for follow up with a lifestyle, dietary, or physical recommendation, medication or lab order, or allows the provider to make a referral. We found that pulling these measures into a dashboard was the best way to keep the provider on track.
A more simplified alternative would be to create a custom orders list with orders that could be checked by the physician. These would be the SNOMED codes for the various follow up measures.
