Has anyone else had an issue where there is a patient in the demoninator of a measure multiple times with only 1 office visit/encounter for that particular period?
We are having this on several of our CQMs.

MU Stage 2 Core 10 (Lab Results): CQR counts total number of Lab Report documents in the denominator. This measure will double-count a document in the denominator if it has been signed more than once.
We have a lot of odd/strange behavior with the CQR - I suggest reaching out to GE to at least get them aware of this. We also saw (we've recently done a total reload that fixed this) "patients" in the denominator with no name/DOB/gender. Thankfully most of the time the PID was there so we could run a crystal report to find the patient - we did see some without a PID though so I imagine those were completely lost souls.
Let GE know for sure though, they may be able to fix it or at least look into it and it will be fixed on the next release (maybe)
I have a ticket open with GE about this double-counting issue with CQR Core # 10, but this ticket is also focused on my disagreement with the way CQR calculates the measure...in short, CQR functionality does not match either GE documentation or CMS definitions. I think GE is more focused on this issue, rather than the double-counting of denominators at this time.
