We have previously met and attested for Meaningful Use Stage 1 back in October. At some point between then and now the report, and only this report, is not calculating properly. Previously we had nearly 100% but now we are lucky to get 14-15%. We are using a custom form using the SMOK STATUS observation term with the ONC-specified values. The odd thing is, if I use the Risk Factors CCC form it calculates properly. Kind of stumped here. The only thing that we did during the time period was upgrade CPS to SP3. I have the updated reports that were posted 1/30/13.

Which other form(s) are you using to capture this data? Since the Risk Factors-CCC form works, it pretty much makes it a form-issue. Any custom forms? Do you have your FH-SH-CCC form populating your ObsTerm with incorrect values (that one can be overlooked)?
Back when the main change to ONC-specified values was made within GE, I created a custom report to show all incorrect smoking statuses to help narrow down rogue issues and non-compliant employees. Perhaps it will help you: https://centricityusers.com/?page_id=.....38;ret=all

Every form we use is custom...I did end up finding out the issue...around that time I had added a period after each of the options and apparently that was all it took to screw everything up.
