We are receiving credit in CQR on measure 3 of the HIE objective (Clinical information reconciliation) when we are not receiving a CCDA to reconcile. (MU stage 3) Should a CCDA be reconciled in order to receive credit, the guidance is vague and conflicting.
Staff are reviewing problems, meds allergies and checking that they were verified in our Problem/med allergy form.
I read the CMS guidance which states: "for Measure 3, the process may include both automated and manual reconciliation to allow the receiving EP to work with both the electronic data provided with any necessary review and to work directly with the patient to reconcile their health information"
The Quality Reporting Guide states: "Note. The CCDA must be reconciled within the Transition of Care encounter document or an append to it to be counted in the numerator."
The Quality Reporting Guide also states "Alternately, you can manually check the medication, allergy, or Problems review checkboxes on the chart summary or Vital Signs - CCC form."
