Working for cardiologists, we do not refer patients outside the practice. How are other handling with in relation to the HIE measure for MIPS?

You can send the TOC to the hospital for pre-op records instead of faxing them. Also I believe CMS will let you use your internal providers to count the measure as long as you send it through the certified electronic solution your using. This will at least get you credit for the base part of the measure even if it doesn't get you many performance points.
Other options are transitioning back to primary care provider, Long Term Care (such as Nursing Homes), Home Health providers and other rehab facilities.

We are working on this right now and not having much luck. All of the external providers that we typically refer to are not in the Surescripts directory for direct messaging and they don't have an email address that we can send to. It is ridiculous that CMS is making this a required measure. The healthcare world is just not ready for this yet.
