We are currently working on our immunization registry interface to NYSIIS through Qvera and the state is telling us that documenting the mother's maiden name is a required field. What do people put in that field if the patient refuses? I can see this is important for children but the state wants this for all patients and I can't imagine too many older adults wanting to give that information?
We request the mother's maiden name when families complete their new patient paperwork to enroll. All of our enrollment happens in the EZAccess Patient Portal, so when they complete the NYS DOH form, we have a line at the bottom that reads: "Child's Mother's Maiden Name (Required for State Immunization Registry):" We have that linked to an obsterm: MOMMAIDENNAM MLI-45055, and while that doesn't populate in patient registration (wish it would), we can see it in the flowsheet. Our staff pulls this information when a new patient enrolls and enters it in registration. This could also be added to your paper forms if you are still doing paper registrations. We have had no hesitancy with anyone filling it out probably because we are asking in conjunction with a state health department form. (It comes across as it's not US asking... It's NYS asking.) If they refuse, you can probably enter DECLINED, but I think if you ask for the information in a way that makes sense to people, you won't have any issues.
