Is anyone meeting this measure? If so, how are you doing it? Our hospital has it's own requirement of 60-65% for the clinics which several of the doctors were meeting or close to meeting. Athena did an update to CQR last week and now nearly every provider is at 0%. We are reporting for 90 days (starting Oct 3rd). Most of the CCDs we receive don't have anything useful and but the providers do click the 'Mark Reviewed' button. Before the Athena update, we could link the office visit note and the CCD and have it count as 'met'. Apparently, linking won't be considered any longer. Also some of the CCDs were not being 'recognized' and did not appear in the Reconciliation form but the patient was in 'unmet' anyway. Patients are added to 'unmet' if the CCD is imported into the chart AFTER the patient is seen in the office (and before there is a 2nd office visit).
This isn't the first time that we've been meeting a measure (Patient Education handouts comes to mind) and then have to try explain to the doctors why they suddenly aren't meeting it because of 'updates'.
These are the steps I been having the providers follow:
!) Mark as reviewed the Problem, Medication, and Allergy lists
2) For all CCDs with a date on or after 10/3, reconcile the items (click Mark reviewed)
3) Check the "This is a Transition of Care' check box
4) Check the "Sign clinical list changes" check box
If anyone has any ideas, I would appreciate hearing from you.

I believe most, if not all of us, are in the exact same boat. There is a post from Tim on the athenahealth portal in the quality reporting group. It states they are "investigating options and will report back. "
I am hoping to hear something soon as I am not quite sure at this point what to say to the providers.

I found out this morning that for some reason the Reconciliation form was not updated during a hotfix update back in July. So we've been using the wrong/old form all this time. Athena is trying to figure out a solution - the the continuity of care documents that have already been reconciled can't be re-reconciled...it's a mess. Thanks for pointing out the reporting quality group.
