Since the measure was changed from "direct/secure email access", how are others documenting this measure since part b is not tracked in CQR?
- Are you keeping logs to document your numerator?
- Are you faxing the TOC to the referring Provider?
- Still sending TOC via direct messaging and keeping log?
- Mixture of both?
Would love some insight on how other offices are tracking this!
I am using Surescripts portal and I am getting numbers in CQR for 15b.
Now if you use any other means besides the Direct/Secure messaging then it will not track. But it is tracking my direct messages just fine.

My problem continues to be NONE of our receiving doctors have a direct email and now they won't even give us a generic email.
Yes that is an issue. Our hopes was as more facilities moved toward MU2 this would get better. We squeaked by last year and probably will again. This is not an easy one to achieve and its not in the control of the provider.
We are sticking with the Qvera workflow and will monitor closely. Any providers that are not (or are in danger of not) meeting the exclusion and are also not meeting the measure in mid December - we'll generate some TOCs the old way, save them to PDF and deliver via other means to get them over the hump. We are using custom reports to attest for the functional measures so we can track this. In theory 15A in the CQR should also track this but since we've moved to a custom solution I don't know how well the CQR is tracking that measure at this time.

Cecil, I've seen inconsistency with data crossing to CQR and using SureScripts (from EMR Oracle). Can you expand upon the exact steps in your workflow? I've been using three:
1. Create CCD-A from within Orders Tab on InProcess Referral. Attach Order document CCD from Messaging Tab.
2. Send email from Messaging Tab but with the SMPP CCD-A attached.
3. Send email from Chart Encounter Form (linked to referral) with CCD-A attached.

1. All referral orders need to be marked as TOC in the orders setup in administration.
2. Order a referral order
3. From the referral order (under orders) do save & create then print or save to chart
4. In messaging start a new referral message, insert the direct/secure email address from your address book, attach newly created Transition of Care document, and send.
Currently the form SM-TOC is broken and is not attaching the order ID in the MUActivitylog table. So it can not be used at the moment.
