Has anyone experienced challenges with the 'new' October rules on Medication Reconciliation? The rules now state (and CQR correctly picks up) 'new' visits where the patient has not been seen by the provider within 3-years. Previously, the measure ONLY tracked the TOC/CARETRANSIN checkboxes and was left to the provider's discretion.
While I can't really explain provider visits where/why they do not address medications, it's become an issue in some situations (ex: maternal fetal) .
Since the rules have changed, the option seems to be going back and reviewing the original notes. While I understand the MU intent, many specialists have always disagreed with Med Rec in general.
