We have not implimented our workflow yet for Medication Reconciliation yet my CQR is reading that I have providers at 100%. We have users on Stage 1 and Stage 2. GEs reply to my question was that by checking the Meds Reviewed box, we are stating that we reconciled for stage 2. How is that possible since the reconciliation button was never pushed to view an external document?
To our understanding the external document is nothing but a confirmation that something was sent to the provider and in no way does CQR pick up on that itself. CQR really just seems to look at if the patient is a new patient and meds review is checked, or the check box is checked at the beginning or end of update to indicate encounter is toc and meds reviewed is checked. We're experiencing the same thing you are and by looking at some of the numerator charts that seems to be the way they've been counting.
Reconciliation in this context does not require electronic reconciliation or extraction of Meds from a CCD. It's all about the clinicians checking the box:
In the case of reconciliation following transition of care, the receiving EP should conduct the medication reconciliation. The electronic exchange of information is not a requirement for medication reconciliation. The measure of this objective does not dictate what information must be included in medication reconciliation. Information included in the process of medication reconciliation is appropriately determined by the provider and patient.
