CQR was down the end of last week for a couple days work.
Func 2-Obj 7, Red Rec at TOC now seems to be showing all my doctors suddenly not meeting this measure. Anyone else notice anything odd?

I have not yet reviewed all my measures - but that one jumped out at me.
I am hoping it is a mistake on GE part with whatever was done the end of last week, and not that all my doctors are suddenly all failing.

All my Medication Reconcile's went to "0" . There is something wrong somewhere because my doctors were 95% and above. The TOC's have dropped also. On those, the Denominator went up and the Numerator stayed the same from the month before. We struggle with TOC anyway because many of our specialist don't have secure emails so we watch it closely.

We are also suddenly not meeting the medication reconciliation measure after the CQR upgrade. In checking visits on some of the patients who are listed as unmet in CQR, all had the correct obs terms in the clinical list changes on the encounter note. That is not surprising, as med rec is part of our workflow when rooming all patients. We have dropped from nearly 100% to 33-51%.

I haven't found any providers who aren't meeting this, yet, put the precentages are definitely reduced. What gives GE?

Our Med Rec numerators are down significantly since last week as well. Oddly, the patient education numerators have drastically increased. We need GE to weigh in quickly as our providers are waiting for feedback on the numbers this morning, but I do not believe any of it to be reliable at this point.

I have the same problem. I put in an SR and this is their response:
ISSUE: SPR 68895 POST CQR Update 1.5.7.1/Meaningful Use/Medication Reconciliation Measure
ACTION: GE has tied this to SPR 68895 and stated the GE engineering team is working on this. I will continue to follow up with them and keep you posted as soon as I hear something on this. I went ahead and tied your practice to this SPR in the meantime.

According to GE:
"All, a fix will be deployed to CQR this evening to fix the Med Rec issue."

Has anyone else seen the changes to Objective 6 that I mentioned above? We submitted an SR, but it appears that GE has only identified a fix for the Med Rec issue.

I re-ran our numbers today and can confirm it's fixed for our site
