A couple of my doctors are not making the measure, and when I looked at the Insight tab and then researched to the patient level, it looked like there was not an 'Activity Log Event' for 'Patient Granted Chart Access'.
Those patients that CQR thinks meet the measure (you can click on the 'Met' #), show this entry. But, when I looked up patient that CQR thinks missed the measure, that Activity Log does not show. However, I can go into Logician, and see the creation of the Patient Portal like for everyone else. Thus, patients are being skipped.
Thus wondering, is there something else needed to meet this measure? Or, should I be looking at user setup for permissions or other settings? Just guessing at what could be different for these doctors.
CQR seems to count the existence of the patportalpin obs for that patient, any entry seems fine as long as it exists. We had an issue where even though we offer it to every patient the portal was only creating a patportalpin entry for those that said yes (Honestly.. probably the proper thing), but CQR wants to see a entry there for a negative answer also (we offered the access, Measure 1 just says we have to provide it, not that they have to use it). You might start looking there.
I can see the creation of PATPORTALPIN, but as I investigate further, in the MUACTIVITYLOG file are various entries. Entry 510 "PatientGrantedChartAccess" is there for a lot of the PIN's, but is missing from some.
And it is these that are missing that concerns me.
So, how and where in the GE Logician world are the entries made to MUACTIVITYLOG?
Make sure all your services are running. We had stored procedures that was supposed to upload certain activities nightly to the CQR that had stopped running. After months of going around and around with my findings not meeting what CQR showed they finally figured out the problem. Once those were restarted which required a patch or sp to the December 2015 CQR upgrade our numbers for Portal as well as a couple other measures corrected themselves. We weren't getting any credit for declinations or PINs generated. We only got credit when they were actually registered and verified directly to the portal while in the office. GE can help you to determine if it is running correctly; sorry that is more technical than I can get and beyond my system access but may be why you aren't seeing what you should be.

As of this morning, a recalc for the 4th quarter now has (as expected) all my providers meeting this measure. Wonder if GE found a bug (I thought they were looking back a full year for this measure vs. only 90 days)?
But I am simply hoping no other bug fixes for this measure so I can get thru the last days of the year and then report for MU.
Although, would be curious to hear what the fix was....
