PCMH was a big push a few years back, and there were state (and other) programs to help medical groups attain PCMH certification. Seems to me like this push, and perhaps the money (??) incentives are going away.
So, are groups still certifying annually (I think that was a recent change from prior 3-year cycles) and/or still qualifying for financial incentives for attainment?

We are - but as an FQHC it is a requirement for us.

If anything, I think it's gaining ground in NY. Ours in annual in NY. Private insurance carriers in our area have been meeting to discuss the possibility of additional incentives for providers who are PCMH certified. (We are a private practice.)

We are level 3 PCMH and maintain annually. We receive a significant amount of money quarterly from one of our payers. We are a private practice in Delaware.
