Is anyone participating in PQRS through GE for CY 2014? Encounter selection for PQRS this year is not based on Office Visit document types. The providers are credited through service orders (CPT codes). If the orders module wasn’t used, the documents for Medicare Part B patients need to be appended with an order to capture a patient encounter using an E&M code or SNOMED-CT code. The will be very challenging as many of our providers don’t enter charges in EMR.
Does anyone have an suggestions on how to handle this?

It will take a bit of form development to help this process, but I would suggest creating some SNOMED codes and then ordering them using MEL_ADD_ORDER as the provider fills out the chart note. You'll have to be sure to make the SNOMED codes have a $0 fee, so the billing side doesn't start seeing a deficit on the account.
You could also make some quick text to create the SNOMED orders with MEL_ADD_ORDER and have the nurse or provider make sure this is done each visit.
We don't use the orders module for billing so we created a button that will push a visit code to the orders module. The physicians and mid-levels are the only ones that see the button and they are forced to click it before they can move off the first page of the encounter form. Once the button has been clicked, it goes away. They hate it but it works and is consistent.
What I find very irritating is, just as KMahoney pointed out, is all of the visits for Medicare patients, since January will have to be appended and add the visit/SNOMED code. Why GE took such a drastic change to how these are calculated AND DIDN'T BOTHER TO LET ANYONE KNOW BEFOREHAND is beyond me. PQRS is not like MU where the reporting is only for a 3 month period. It is for the whole year! From this point on we can adjust, but the work that it is going to take to go back and append and add the order is going to be very time consuming.
how about the fact that you have to wait until service pack 7 to remap all insurances to patients... by the end of the year... and it is into November and no service pack 7 is available yet.
Also still don't completely understand what is required since the "reference guide" that is supposed to explain all of this in detail is no where to be found. Something about Source of Payment code.....
Just an update - we will most likely be going with an outside PQRS registry to report our PQRS measures. We will not be able to upgrade to 7, add snomed codes to our visits and implement CQR in time. This is all very unfortunate.
