Hello,
I recently built a quick PQRS template for our physical therapists to record that information on. I pretty much just copy and pasted from our PQRS measures from our other providers which do report correctly and I mostly built in house. Anyone have any idea why they wouldn't be reporting for the PT on my completely custom template? thanks

PQRS for PT is based on different E&M codes, so maybe that is why you are not seeing results. Most measures for PT are based on 97001,97003.
