Good morning
We are working on becoming a patient centered medical home and have begun pre-visit planning. Wondering if anyone has a form that they have created for this purpose that they would be willing to share.

We designed and built a form for the MA's to review some key measure data prior to the office visit. Perhaps this will give you a start or an idea of what you might begin with.
