Anyone having issues with this one being counted? We have been doing exactly as told, using CCC FH form and populating History for first degree relatives, but are not showing any met in the CQR report.
I'm not but we also aren't using the CCC form. Could you be using an old version of the form? Could there be mapping issues in administration? (I don't believe so though, I think this one is pure obs terms)
yeah, we are using the latest version published with service pack 10, but not the version with ICD fixes.
I dont think it is a obsterm only, but it should be populating a table, which I suppose CQR pulls from, via the CCDA that is pushed out??? The processing has gotten so convoluted... especially when GE changes the name of the web service directory being used by QIE... by I digress.
Remember that 1.) this measure has gone away 2. ) for counting on CQR, you need to have had SNOMED codes attached to the Family Hx entries, which the form did not always do out of the box. Those can be fixed retrospectively via SQL.

FNC_2_Menu_4 = family health history
Do you have a source document for that? I didn't see this.
On a GE page for CQR (clinical quality reporting), was a screenshot post of the CMS ruling. I just tried to convert the text to the CQR Measures.

The FH measure is not a part of the Final Rule. Relax everybody.
