VFE form for PQRS that populates flowsheet???

Please query CHUG community for a VFE form for PQRS. Would like a form that enters the data into flowsheet and is easier for the nurses to collect the data.

Anyone have anything like that???

I have a VFE form for a Cardiology group that addresses this issue. It is basically a roll-your-own patient PQRS dashboard. If you are interested in the details contact me directly. Form looks like this:
