No one has been able to tell me definitively whether or not there is a minimum percentage required for the clinical quality measures. From what I've read online it doesn't appear a minimum percentage is specified and that the real purpose of including them in MU attestation is to show that we are capable of reporting clinical quality measures. Can someone confirm that?
My reason for concern, for example, is that I just ran some reports for 10/1/2012 - 11/5/2012. On of the clinical quality measures one of our providers had a 0/2. Zero percent. Would you get credit for attesting for that clinical quality measure even if it was zero percent?
Also, I've been told that you must pass every single measure to receive any of the incentive payment. Is that correct?

There isn't a threshold for the quality measures (yet). You just have to report something. If you have a 0 for any core measure denominators (i.e., 0%), you have to select an alternate core in its place. You can still report a 0 denominator for the alternate.
You have more flexibility in the elective measures you can choose for Stage 1. A lot of physicians chose measure that weren't applicable to their specialty and reported 0%. In Stage 2, you have to try to select quality measures relevant to your specialty, though your percentage could still be 0%.
