We are trying to update the Social History-CCC HTML form. Want to add the sexual orientation (PATIENT.SEXUALORIENTATION) to pull from the patient registration to the SH Template. We are able to edit the social behaviors on the form but cannot edit the SH Template.
Has anyone had success doing this?
