I am using this in a data display but what I get for return is:
Return to clinic in 1 week
Return to clinic in 1.
Return to clinic in
Not sure why the return is that way. No translation but can't make it stop displaying that way.

Have you verified the Obs Values in your database for the Obs Term in question? The issue might be that it is recorded like that and that you LastObsValueByLoc is working correctly, but that you need to fix the form that writes it to the Obs Table.
