Anyone ever created their own OBS terms instead of going through GE and requesting one?
Alas, to the best of my knowledge, this is impossible. I have had good experiences requesting new obs terms, reasonably fast turnaround. If this an issues, suggest looking at the obs table, picking a set of terms that are never used in your practice, or identifying obs terms that are "close enough" to what you want.
I am normally successful finding an OBS term not in use at my practice, and using that. As you can through the list of all OBS, you will quickly discover lots of codes created for things that you know you will never do.
That way, I can continue developing forms without waiting for GE to make the code and a new kit to become available. Often, nearly exact matches can be found for most of my needs, and I 'compromise' with one or two codes. But, the new form is in test much quicker.
I suppose, if you have a large project and a longer development cycle and lots of very specific codes, then sure request.
It's not impossible, we've created our own using an insert command in SQL. Just had to make sure we created in own HDID(among other things). Just curious if others out there have and what impact future upgrades have had on OBS terms that were added by clinics.

Modifying tables directly in SQL is not for the faint of heart. Keep in mind, as you migrate down the path of endless version changes, the tables would have to be changed again. GE would go apoplectic if they knew you were directly modifying tables. Like they say, "The warranty is null and void if you pry open this box." I agree with joeg1962.
