I created a form as part of our hypertension control project and wanted to know if there is a way to calculate a score based on patient answers. For example, if a patient answers yes to questions each question is assigned a value of 1 whereas patients who answer no to a question the value of 0 is assigned. At the bottom of the form I would like to have a calculate button which would give the total score. Thank you in advance for any advice or input.

A simple old depression scale I used to use. This should help point you in the right direction.
