is there a way to add the note to pharmacy to MEL_ADD_MEDICATION or any other way to pouplate that field programmatically?

Currently, no, not that I am aware of. I would suggest making it an enhancement request for the MEL_ADD_MEDICATION symbol to the athenahealth team. Seems odd that it is not already part of the function.

the note can be placed in the instructions, though those are limited to 140 characters and some instructions are already very long!
