We are a new Phreesia customer and I am trying to bring data gathered by Phreesia thru their clinical intake into our EMR and then into an office visit.

Does anyone have forms they are using for this they could share or could make suggestions?

We have done a lot of integration with Phreesia for clinical data. We would like have some templates that we could use as a starting point. You would have to get Phreesia to configure a results interface, but they are very good at this. let me know if you need some help with this.
