Can anyone who is currently allowing patients to fill out e-Forms on Kryptiq's Patient Portal for things such as ROS, PMH, and Risk Factors provide some insight on their workflows? Specifically:
- Do you have these documents come in as unsigned or signed documents?
- Do you map their answers to "Pt. Reported" obs terms or the same obs terms captured in the OFV?
- Did you create a form to populate the patient's answers to be reviewed by the Provider prior or during the visit?
- Did you find that allowing patients to fill out e-Forms streamline the check-in/rooming of the pt.?

I have worked with groups that allow patient's to fill out e-Forms and even sign them from the portal. They are automatically indexed into the patient chart. The documents all come in as signed. A secure email message is sent to a proxy users desktop to notify staff to go in and make updates. This particular office didn't want anything to flow right in and update. They wanted someone to manally review the choices and then do an EMR clinical list update.
At this time, they do not have any OBS terms mapped that I am aware of. However, I do know of one office that made an OBS term map back in the registration. That OBS was sent to the EMR banner that the patient did, in fact, register on the portal. I thought this was clever because there really insn't much available reporting on that currently.
I have found the information they get from the patient is more complete than when patients fill out the paperwork in the office because 1) patients are at home and can take thier time. They have access to all thier receipts and records at home. They are giving more accurate dates. 2) you can default a lot of the demographic information so the patient doesn't have to fill out as much. If they do make changes, you can see the "before" and "after" when the document is sent back in the secure message.

I attended your presentation at CHUG, but I, like Alice, would like to hear back from other clients who are using the Kryptiq portal on how they specifically are using the eforms that are available.
One thing that we are a little disappointed about is that, within the eform, we can't present what we currently have for problems, meds, allergies so for the existing patients who are coming in for an annual visit or maybe to a different physician they haven't seen before could fill out a form and just update what has changed.
